I'm not quite sure what he's looking for.
He wants protection from scrying is clear.
He wants the activation of this to be dropping a piece of paper with his true name on it (paper as a generic term, or specific? I.e. would parchment work? It would be be more appropriate) into a hat.
This seems like a magic item to me, more than a spell, one that will protect you from scrying for a duration while your name is in the hat.
The last clause confuses me. "And then a mystic alarm telling him the true name." Does that mean he wantts something that will tall him the name of anyone who scries on him? That is generally very hard to do... there aren't many effects that will give you someone's true name.
